CAUTION: If the wound is more than four inches below the knee or elbow, initially position the tourniquet band two to four inches above the wound. The tourniquet is applied by placing the loop two to three inches above the injury site and pulling the excess material tight through the buckle. However, there was no significant difference between the three tourniquets in their ability to maintain pulselessness after subject maneuvers. If there is still a pulse below the tourniquet or if arterial bleeding continues, tighten the tourniquet. A tourniquet is applied only if bleeding from an upper arm, forearm, thigh, or lower leg threatens the casualty’s life. In successive trials, subjects were timed during the application of each tourniquet to the upper and lower extremity. Preclinical graduate health science students were randomly assigned a commercially available windless tourniquet: SAM XT, Combat Application Tourniquet (CAT), or Special Operation Forces Tactical Tourniquet (SOFT-T).
